ROE Form — Download, Fill and Sign (Canada 2026)
The Record of Employment (ROE) is the form Canadian employers must issue to any employee who stops working — whether they quit, were laid off, took leave, or were terminated. Employment Insurance (EI) depends on it. Here's where to download it and how to fill it in.

- 4
Issue to the employee and Service Canada
Employees need their ROE within 5 calendar days of their last day. ROE Web files automatically go to Service Canada; paper ROEs are mailed.
Frequently asked questions
Who issues the ROE — employer or employee?
The employer. The employee is entitled to receive an ROE within 5 calendar days of their last day of work.
What reason codes are used on an ROE?
Common codes: A (shortage of work/layoff), D (illness or injury), E (quit), K (other), M (dismissal), N (leave of absence), P (maternity/parental). Full list is in Service Canada's ROE guide.
Can I issue an ROE on paper?
Yes, but most employers must use ROE Web. Only certain small employers can still use paper ROEs ordered from Service Canada — not downloadable from canada.ca.
What is block 15C on the ROE?
Block 15C is the insurable earnings by pay period for the last 53 weeks (or less if the employment was shorter). This drives the EI benefit calculation.
Can I fill an ROE PDF in a browser?
If you have a paper ROE PDF, yes — use our Edit PDF tool. For ROE Web, use Service Canada's online form directly.
Are my files uploaded here?
